I am a housewife and pregnant with twins, and would like to know what are the specific duties of a nanny, and if I need to hire a housekeeper as well? Right now I just have a cleaning lady. I am disabled, so I need some help around the house.

Hire both a nanny and a housekeeper nannies don't want to take the time to do deep cleaning.
