Make it more general, as in "...you know, there's a lot of people out there who have really bad breath, and probably don't even know it...it can be a sign of something serious, like a problem with their teeth...they probably are embarrassed about it, or don't even know it. Now, if I had bad breath, I'd like someone to tell me, so I could do something about it..."

"Oh, by the way, want a mint??"

It is your duty as a friend to tell her. "Look, NAME, I have something I have to tell you something about yourself and you are not going to like it but because we are friends I am going to take the risk and tell you anyway, okay?"
Then just go ahead and tell her. If she is any kind of a friend she will be grateful to you.

send her an anonymous note. Type it so that she doesn't recognize your handwriting.

just tell her ...offer solutions... how will she feel when someone else tells her and you didn't..
