Make it more general, as in "...you know, there's a lot of people out there who have really bad breath, and probably don't even know it...it can be a sign of something serious, like a problem with their teeth...they probably are embarrassed about it, or don't even know it. Now, if I had bad breath, I'd like someone to tell me, so I could do something about it..."

It is your duty as a friend to tell her. "Look, NAME, I have something I have to tell you something about yourself and you are not going to like it but because we are friends I am going to take the risk and tell you anyway, okay?"
Then just go ahead and tell her. If she is any kind of a friend she will be grateful to you.
