second-take your application to your bank to finish it. That's how you get your routing number. You do not need to put down your city, but you need to know your bank. Make sure your account number is correct.
Third-take your application back to work and hand it to a person at finance department.

Yes the best way is to call. Routing numbers change sometime because of mergers, and you do not want to guess at that !
