They won their league championship series; so one American League team wins the pennant and one National League teams wins the pennant. These two teams meet for the World Series.

Before 1969 there divisions didn't exist neither in the National league nor the American so a pennant winner used to be the one that had the best record on their respective league (National or American) but that lasted until 1969 when the playoffs where introduced to determine the respective league champion.
So answering your questions until 1969 it will give you the chance to go to the World Series.
It is called a pennant because the winner in the years before would get a commemorative flag to be displayed or flown by the winning team.
It used to be a term used in both football and baseball, but lately it is only used in Baseball.
