You can just draw one up in a little notebook or diary or on a spare piece of paper.
It is useful to have a list of your medications on you because sometimes you might not be able to communicate to a health care provider and such a list provides invaluable information!

What you need to do is buy a graphing paper. List all your medicines on the left hand-side and after the medicine, write the time of the day when you take it. For example, if you are taking a medicine that is 3 times a day, you can time that for 6am, 2pm and 10pm or whatever is convenient for you. You sign the paper after you take each medicine in that way you don't forget taking your meds. This is an easy way to keep track of your medicines. One more thing, if you have medicines that are once a day, take them all at the same time around 9am or 10. If they are blood pressure medicines, space them. You can take one in the morning and one in the evening. Hope this will help you.
