At my brothers job the other day they had to fill out new job apps because the company was sold. When it got to his lfe insurance, he put his ex girlfriend as the beneficiary? Why would someone do that? She threatened to have him beat up and everything. Is he in denial or what?
I know it's ultimately his choice but as his sister, I don't understand.

I would definately question him about this, surely you family is more deserving and who is going to pay for his funeral expenses?

Sounds like your brother is having trouble with reality. A few things to do.

First, get him out of the house (especially if they lived together or spent a lot of time there). Try a stupid movie or coffee house/club they did not go to as a couple. Get some of his pre-relationship buddies to take him out too.

Second, listen. If he talks (and don't force it) listen. He needs a safe person to confide in. Remember: listen, don't judge. Even if she's a total you know what, never say it. Just nod, listen and console.

Third, encourage. Let your brother know he's still loved and that there is sooo much out there (keep in mind no bashing the ex).

He'll heal and your relationship will be stronger.
