Well, according to Chris Rock, "If a woman has a pierced tongue, she'll probably suck your d!ck." Doesn't sound lesbian to me! Also, what self-respecting lesbian wants to risk a tongue infection?!
I actually have only known one lesbian, and several straight and bi women, with pierced tongues.
To me all it says is she doesn't care about her oral health or whether she breaks her teeth.

Neither - just slaves of what they think is fashion, and they end up lisping and clicking and grossing people out. Tongue rings shriek "low self esteem." And the way other people react to them reinforces their sense of worthlessness, so they go out and pierce something else dumb.
