Japanese professional wrestling, or Puroresu, is, like American Professional Wrestling, an entertainment spectacle in which the outcomes of matches are predetermined. However, there are some very distinct differences.
While the majority of American wrestling is presented as a larger-than-life extravaganza, with intense interviews and colorful characters, Japanese wrestling takes a more realistic approach. Think of it in terms of movies. If American wrestling is "Batman & Robin", Japense wrestling is "Batman Begins." The interviews are conducted, generall as if they were competing in a real sport - very little yelling, screaming and "OOH YEAH BROTHER I'M GONNA BODYSLAM YOU AND TAKE YOUR TITLE!"
Also, the technical aspect of the matches are emphasized as opposed to power moves (although there IS a segment of Japanese wrestling that is composed of just non-stop extreme violence). And while the OUTCOME of the matches are predetermined, participants in matches will generally improvise their moves, or "spots", as the match goes on, relying on cues from the referee and their opponent to determine preplanned events, such as when the match should end, or a "twist" in the match (such as someone interfering from the backstage area).

Professional wrestling in Japan is commonly known as Professional wrestling. Many Japanese wrestling groups have seen a significant downturn in popularity in the past decade due to a failure to introduce new talent and copying many unpopular ideas from the U.S.

If you mean so called "PRO-Wrestling" in Japan, everybody knows it is a show, scripted.
But K-1, PRIDE and other Karate or Judo related Wrestlings are real. These are called "ultimate fights".

It's also scripted but the thing is that they go for more of a technical approach and pure wrestling, and not so the showmanship or soap opera of the american wrestling.
