in order to take care of the others he needs to take care of himself

Get him help. he is in a downward spiral to rock bottom. But, help will only work if he wants it. It may take rock bottom or something really bad happening to him before he wakes up and puts himself first.
If you want to understand this you can go see a therapist. Trust me, it's serious for most people.

Eventually things he takes care of will fall apart...then you can give him that example and tell him if he was more organized this would not have happened.
