It could mean many things. Like: its the lies we tell ourself that are the most damaging. Or: we may be telling huge lies by keeping our mouths shut. Or: we allow others to continue in their ignorance because we're afraid to share our thoughts and feelings.
Personally, I'd rather have people tell me what they really think than to play pretend about things- even if the truth is a little ouchy. At least I know I can trust them.

It means that sometimes it is worse to keep your mouth shut about something than it is to actually tell a lie.
When someone says something to you, you can decide whether or not they're telling the truth. It's up to you to make a decision. However, when something is going on and no one says a word, you have no power because you don't know anything.

Not telling somebody something that they should know. Like not telling your girlfriend that you are married. IF you don't tell her, you hurt her more because you didn't give her the choice of if she wanted to be there or not.
