Let's assume that he'd cleaned himself up by the end of the 70s (very important, else the rest of this is academic - the man was in BAD shape when he died!)
Sadly, I think he would have been largely a musical irrelevance in the 80s,90s, 00s. Revival of a couple of his hits in commercials (e.g. 'A Little Less Conversation') would have given him a little lift (after all, Levi's boosted sales for both Percy Sledge and Ben E King) and allowed him to mount a mini-tour in medium-sized venues.
He'd have ended up doing a Far East leg, playing in the same places that hosted the Platters and Englebert. Later on, for shock or novelty value, a really hip young band would've sampled an old Elvis hit, then paid him to appear in their video, most likely a cameo comedy role set in a fast food restaurant.
Near his end, like his close contemporaries Jerry Lee Lewis and Carl Perkins, he'd have been making duet albums with modern musicians who, in a borderline-patronising kind of way, always want to attach themselves to a legend before he dies.
I'd rather not watch. I'd rather just remember the vital, charismatic performer who cut those six or so classic Sun singles that both made rock n' roll possible, and put the sex into it.
RIP, Elvis, hope you really are dead, and resting well.

Doing his drugs and getting fatter. I loved the man but he really did let himself go. But then again with the way times have changed he may be exercising and getting himself healthy once again.
