I asked a similar question a couple of days ago and have downloaded at www.finale.com. It's a great simple programme to use.

Not any good free ones.

"Band in a Box". Not cheap.

Will generate notation and chords from a midi.
One can also manually enter both.

There is a new one, but I'm not familiar with it.
Finale allows manual entry of notes.
CoolEditPro permits mixing from wavs.

Finale has a freeware version.
