I second stahls.com - they have jersey kits.

If you cannot get what you want there, try finding someone local who does machine embroidery. Our local stitch point place can embroider nearly anything you want, judging by the people around town with their clothing on. (We're a very small town, but nearly everyone has something with the local high school logo on it.) I know that they can make patches that you could sew on.
