The laying on of hands happens most importantly in Confirmation and Holy Orders (ordination).

Laying on of hands can also be used in times of prayer. It is an act of connecting with the person and God as you pray for the person. It is feasable that in some churches there is laying on of hands when a person is baptized, as others pray for the person. Generally, it is not considered a SYMBOL for baptism, though.

The laying on of hands belongs to confirmation and holy orders.

No. The laying of hands started when Jesus laid his hands on Peter and gave him the name. Every bishop has followed in this line.

No, laying on of hands shows approval. Also acts as a form of desposit from one person to another. Scripture shows that it was an act of passing on what you have to another person.
