Originally, a bohemian is someone from Bohemia, a region which is today (more or less) the Czech Republic. Later on, especially in France, it came to mean "Gypsy", probably because they thought that all gypsies came from Bohemia.
Much later still, by Bohemian, they meant an artist, an intellectual or someone who lived an "artistic", alternative lifestyle.

Subject, Tone and Attitude at the time they say it, will give you a better idea to the meaning: For ex: If they are talking about someone they don't like, a "bohemian" lifestyle would kind of an insult--(low life) But if they liked a person and find him/her unique and interesting, it would be giving them a high praise.
