It's a disorder, which is a psychological condition. You don't choose those.

Eating Disorders meet all the criteria for a legitimate mental illness:

1. They involve mental events and behaviors that are disturbed and abnormal. Although they are different from normal people, they are "stereotypical" which means that they are the same from person to person who has the disorder. These really aren't the "choice" of the person afflicted, although they may rationalize it as such. The disorder interferes with their choice.

2. They hurt the person.

3. They are readily diagnosed.

4. They can be treated effectively and the treatment is the same from person to person. In other words, you treat it the same from person to person and the treatment is effective.

The bottom line is this. Eating disorders will hurt you and won't go away by themselves. That is the bad news. The good news is that most people can be successfully treated.

It is a disorder because a choice would just be to lose weight, but some people with eating disorders lose a lot of weight and become so thin and ill-looking and think that they look so much better this way. There is a disorder in the people that have anorexia or bulimia that makes them try so hard to lose weight when they were probably never overweight to begin with.

An eating disorder is a psychological condition with underlying pathological issues. It is not merely about not wanting to eat. A person may exercise control over food intake because they feel they have no control in other aspects of life, they fear gaining weight, body image issues are intensified...it is not a choice, it is a disease!

It starts out as a choice, but it becomes a disorder.
How and why is in the girl's mind. She thinks she is fat, so as she looses weight she honestly doesn't see it in the mirror or on the scale. The scale may say 98lbs, but she sees 160. She has conditioned herself to think that she is fat, so that's what she sees. Even loose clothing that used to be tight won't break through the barrier that gets built up. It doesn't happen on purpose, but it does happen.
When someone doesn't eat, that's called anorexia. Bulimia is when you eat, but then force yourself to throw it up.

Eating your daily needs is a choice. Not eating at all because you don't feel like it is a disorder.
