listening - we failed to listen, we do not listen 'attentively', we hear what we want to hear.

sender/receiver barrier - intended message is distorted when received by receiver, due to above. To overcome this, repeat and clarify, and seek confirmation that message is received the way you intended it to be.

communicating - not enough clarify (words used/misused, details etc) leading to miscommunication. To overcome this, repeat and clarify, and seek confirmation that message is received the way you intended it to be.

Have you ever played the game telephone? where 1 person wispers a phase to another who wispers to another and so on....when you r done, the ending phrase does not match the first. well it is the same idea...people hear what they want to, or what they think they heard, or saw for that matter....several people see the same accedent, and all accounts are different....miscommunications happen the same way...people r unrealiable sources of information...

I think mis communications happen when people look further into what is being said or they don't clarify what is being said, or they don't listen to what is being said as they have a pre conseved idea of what should be said or what should be done or it goes against their beleifs.

it could be from the lack of forethought or even a faliure to give proper information in the first place.
