What kind of vehicle is it? Make? Model? All info would be most helpful! Rear wheel drive? Possible pinion noise? A better question will get a better answer.
Update: Scratch the pinion noise then, and thanks for updating your information, it makes it easier to help. It could be a gear noise from the transaxle, I have seen this on the Dodge Stratus and Neon. I work at a Chrysler/Ford dealership. It is a normal gear noise on these particular vehicles, but keep an ear out for any increase in volume. If it becomes louder and more prominent, have it checked by a professional for potential transaxle wear concerns.

Alot of newer front wheel drive 5 speeds have 5th gear isolated from the the rest of the gears. Often the only way that 5th gets lubricated is by oil being thrown onto a trough in the transmission. and then running down the trough onto the grears.
5th gear will often be the first gear to fail when the fluid level is low.. The reason it is noisey only on accel is because that is when the most load is being placed on the gears. They overheat and get noisey, eventually breaking apart.
Get the fluid level in your transmission checked right away.
Also, FYI, it can't be the clutch because you said the noise is in 5th gear only. A clutch does the same in any gear.........

i own a repair shop,,and i think what your hearing is a air intake sound, that some cars make ,,when you accelerate them,,you said it made no sound at regular speed,,if it was a bad bearing in it,,it would do it all the time,,it would also have a slight roar in it,,,,and you don't have a clutch going out either,,if people would only answer questions on here based on what they actually know,,it would be a lot better place,,anyway,,keep a watch on your fluid levels and see if the sound gets louder ,,or just stays the same,,i think its an air intake sound which most cars do make ,,but like i said if i gets worse take it and have it checked out,,and i don't think you,ll be needing a transmission any time soon.good luck with it.

Hey man. Just check to see if the fluid level is low. If is then top it off. If it is full changed the fluid. Then dont worry about. My car does the same. Been doing it for the last 4 months and its fine.

That is your engine purring. it's a good sound. get to know your car. All cars have a personality. Don't worry things are normal. if anything check your transmission fluid if it is an older car. but if it is new. Your car is purring like a kitten.

You think it's the tranny? do you use 5th a lot? Maybe it's fresh so might sound that way, especially if you are not in 5th too much.
Can't be the tires, right?
I don't hear anything when I'm in 5th, but the rpm's are low so I do get a "hum" but I'm not concerned abou it.

It's either a bad bearing in 5th gear or gears are worn in 5th gear.

How often do you drive in fifth gear? Probably not often, so maybe the gears are still wearing in.
