where can I find a map that outlines all the neighborhoods in boston (Back Bay, Fenway, South End, South Boston, etc.) either a paper copy or on the internet?

I couldn't find one huge map with all the neighborhoods on it, but I did find a page with a map of each one:

There is no map with those on them that I know of (I have several maps of Boston and none have them listed). The neighborhoods are loosely bordered (where one changes to the next etc.). If you are in Boston, bring a Boston Map to a real estate office (say you are looking for an apartment or condo) and ask them to draw them down for you - most Bostonians can do it for you.

Some neighboorhoods are easy to find - like the Fenway is near the Fenway park (park not Ball field) and Beacon St, Beacon Hill is between Beacon Street and Storrow Drive, North End is between the old route 93 and Commercial Street. Good luck.

Try Amazon.com for local guides, because I don't think anything on the internet will tell you. Maybe try to go to the parking clerk, because that's where people get parking passes of each of these districts and so it would stand to reason they have an outline of them. . . .Best of luck!
