You could make a scrapbook of pictures and movie stibs and things that you have done since the two of you have been together.
You could bake her a cake and make her a nice dinner for two.

Mini Notebooks
Materials:
tag board
handmade paper or gift wrap
Post-It pad of any size
ruler
X-Acto craft knife
scissors
glue stick
decoration for top of book such as old buttons, charms, photos, ribbons, beads
Steps:
1. Measure the top, bottom and side of the Post-It pad
2. Cut the tag board slightly larger than the total measurement of the three sides of the board so that it will easily cover the Post-It when scored and folded. Score deeply (but not all the way through) with the X-Acto knife where the board will fold
3. Cut a piece of decorative paper 1/2-inch larger than the cover. Glue the paper in place onto the board while the board is in a slightly folded position. This will allow the book to stay in a closed position when complete
4. Cut the excess papers at the corners and sides where indicated and wrap the corners to the inside of the book. Cut an additional piece of paper and add it to the inside to cover the board
5. Glue the bottom of the Post-It pad and place it in the book
6. Embellish as desired
Five-Panel Screen
Like a wall liberated from its structural duties, a folding screen can be called on at a moment’s notice to serve any number of purposes. It divides space, bars passage, creates privacy, or hides disappointing views. It can also be folded up and set aside when it’s no longer needed. The easiest way to make one is to let the lumberyard do most of the work.
The screen at left is constructed of five 1-by-12-inch stock pine boards. Have the boards cut to any length you please;42 inches tall.
Then sand, prime, and lightly sand the wood again. You can paint the panels different colors.
Use 3/4-inch double-action hinges (brass or brass-plated), which fold in both directions to make the screen more flexible.
Screw them into the boards 12 inches from either end. Rubber-ball “feet” tacked into the bottom edge of each board will protect floors from scratches; place them 1 inch in from the corners.

You should make lunch/ dinner and put it in a picnic basket and have a picnic. Get a blanket and some flowers and have a romantic picnic someplace by yourself outside. If the weather isn't right for a picnic then have it inside. Just lay the blanket down in the living room turn off all the lights and light some candles give her the flowers and then you guys can have a nice romantic evening. Then for the gift part you can make her a scrap book with like pictures of you guys in it and you can give it to her at the end of the picnic. She'll think it's romantic and sweet.

Jacob. Looks like you between a rock and a hard place, son. Women, Birthdays and 'I don't want anything', go hand in hand, get use to it boy. Sorry ladies, you know it's the truth. What I have done in the past was to get a large handwoven basket. Cheap at any craft shop, not one of those wash baskets. A nice basket, one with a handle. Now, items for the basket. Her favorite wine, liqueur or cider and two glasses. A gift certificate for two to her favorite eatery. A gift certificate for two to a show or movie. A couple of scented candles with candle stands. Her favorite flowers scattered around in the basket and fresh poped popcorn in the bottom. You do this for her this year and you will be in trouble finding something better for her next birthday. Good luck my friend.

U can try to do up a bouquet of her favourite flowers urself. Like buying the flowers, the paper 2 wrap, ribbon, scissors etc. Then end it off with a small bear like forever bear sth like tat at the ribbon part.
2nd part is to fix up a dinner for her and u of coz. With candles and err.. ask ur family members to let u have privacy of the house just for 1 evening.

How about building her a nice vanity that goes with the rest of the furniture? It is a lot of work but well worth it, because women do like their personal space for make up, perfumes, etc.
Or design her a nice desk for the pc with all sorts of gadgets in it(like a keyboard tray hidden in a drawer where the front can be folded over and used as a wrist rest.....Any kind of furniture we build from scratch and of solid wood is valuable and will last for the rest of our life....so that is one idea....

Have someone paint a portrait of you guys or of just her.
Create her a special gift basket with all her favorite.
But her a star and name it after her.
Buy her a personalized bottle of wine.
Make a formal list of all the things you love about her and frame it.
Remember it's the thought and energy that is put inot it that counts.
