How do you recieve a Minor Work Authorization form?

Well, I'm 17 and I just applied for a job and I got called for an interview, but I don't have a Minor Work Authorization form. I don't even know how to get it, and neither does the manager of the place where I'm applying to. I've asked some of my friends and they don't know either. If you can tell me how to get one I would be eternally grateful.

The counseling office of your high school should be able to help you with this.
