The don't technically even have any land except the Grand Masters house in Rome and the Villa Malta, also in Rome. They do, however, issue stamps an have a currency, the Scudo.

The smallest population is Pitcairn Island, where the population is about 30-35. It is the island where the mutineers from the Bounty settled. It has no natural harbor, and there is a shortage of healthy adult men strong enough to row longboats out to visiting ships, so it may have to be evacuated sometime in the next decade.

The smallest independant state is Vatican City. It's entirely inside the city of Rome and occupies only 108.7 acres. It only has about 700 people living there. Pitcairn is tiny, but is a colony of Great Britain, and not a nation or country. They don't even have their own currency.
