In the novel, it is a centipede. A big centipede.
In the movie, it is a tarantula.
In the movie, you see Bond run into the bath, and kneel over the toilet and heave (but you don't actually see him puke.)
An interesting footnote about the movie. In one or two takes where you can see both Connery's face and the spider, you can see a reflection/glare on the glass which separated him from the spider. And, they used a body double for shots of the spider crawling on Bond's body.

OK.I'm a big big Bond fan. There are a few differences between the Novel and the Movie.
NOVEL: It was a SCORPION.
MOVIE: A spider.
NOVEL: He felt sick after squashing the scorpion, goes to toilet and throwsup (Exact words from the book: "Bond was violently sick").
MOVIE: Bond goes into the toilet and closes the door (the producers thought its not good to show Bond throwing up as it may be looked upon as a sign of weekness).

I saw this movie when it first came out. It was a spider and he did throw up. Bond was human in those days.
