Depends on what bad track you are on now..
If you have bad friends who are on the wrong track with you, got you there, or any common sense will tell you they won't help you get out, leave, make new friends.. maybe you should move to a different town, start a new life. Look at what elements in your life are keeping you on the wrong track, and do away with them.
Get a job if you don't have one, change jobs if you have a bad one that makes it harder otherwise to stay on the right track.
Make sure you have constructive things to occupy yourself with, set goals, not too high right away, little ones, and start reaching them. Make a list of small goals, and strike them out one by one as you complete them.
