It seems kind of ambiguous.
Obviously Brian understands him completely as the two often have long conversations and seem to understand eachother perfectly. However Brian has never "translated" for Stewie or given any kind of indication that the others do not understand him while he does.
The parents and meg/chris seem to understand the gist of what stewie is saying. For example "Oh you want your toy back" etc but never his exact words (as I assume they would be somewhat surprised to hear stewies voice)
As shown in the movie, stewie can understand his older self and stu seems perfectly able to understand stewie. However future lois still can't understand stewie. Sometimes side characters (other children "bertand" etc) seem to be able to understand him perfectly. In one episode when stewie talks "through" chris to a hardware shopkeeper, the shopkeeper seems to understand that he wants some sort of power saw.
One odd diversion from this pattern is an episode where peter is attempting to potty train stewie. Stewie says something like "then we can get dunk and light up a doob (marijuana)" peter has a blank look and then says "really?" indicating that he understands the baby.
I think perhaps you can think of it like this. We the audience are shown what stewie THINKS he is saying. This can also be heard by Brian and some other characters like Bertrand and Stu. The parents and others hear some sort of baby talk that kind of matches up to what stewie is actually saying "I WANT PANCAKES" etc.

In some episodes, he IS answered directly, but in others, his family acts like he's just spouting baby speak. YouTube is down right now so I can't give you the link, but there's a fantastic video there of MacFarlane speaking at a Harvard or Princeton graduation ceremony, and he tries to answer the question. It's also entertaining just to hear this slim guy in glasses talking in the voices of Brian, Peter, and Stewie.

Only the dog, Brian, can hear him in the family. Other than Brian and children his own age, Stewie's brilliant rants fall silent to the less intelligent characters that make up his daily existence.

i do think Stewie can be heard by others but not all the time. In the pancake episode Stewie yells, "I want pancakes! Give me pancakes!" Then Meg hands him some.
But in other episodes Stewie threatens Lowis and she seems 2 not hear him. So who knows??

Brian (the dog) can definately converse with him, as can other babies. In 'The Untold Story' he discovers himself from the future, and he carries on conversations with himself, and at one point he (the baby) yells "Let's have sex!" and his (the older Stewie from the future) date answers as if the older Stewie said it.
Other times various people have spoken with him, too.
But Lois and Peter (the mom and dad) do not seem to understand him at all!
Sometimes I am confused too!

Brian is the only one who really pays him any attention and has serious conversations with him. Anyone he has ever had a conversation with has been very brief. I think that Seth had purposefully made it that way to constantly keep the audience in a slight state of abiguity about it.

well, that's a very interesting question.
i've been told by a family guy authority (i.e. my fg-addicted friend) that stewie can be heard/understood by others, but they don't pay him any attention..
but i've been watching closely, and this is what i think:
stewie can definitely be understood by brian (the dog), but no one else pays him any attention, and so it just sounds like he's whining or crying to them.... watch the episodes closely-- see how they react to him, and replace his words with a baby crying, and think if they'd react any differently.. they wouldn't..
and as for all those dance numbers-- yeah, he's dancing.. but he's a baby, and babies dance and ppl think it's cute.

I know Brian the dog can hear him, b/c they talk to each other a lot. For the most part, I don't think anyone else hears him, but occasionally, they do seem to respond to him. Remember, it is Family Guy, so I wouldn't expect them to really care about consistency.

He has conversations with Brian (the dog) and in the movie he had a conversation with the adult version of himself.
But I think you're right, I dont think that he is heard by the rest of the family.
