Make sure machines are anchored securely to prevent "walking," tipping, excessive vibration or other movement that could be hazardous.
Never reach blindly into areas that may contain energized parts.
Be sure there is enough lighting to clearly see all points of operation.
Keep conductive items -- watches, rings, steel wool, belt buckles -- away from exposed electrical parts.
Never plug or unplug electrical equipment with wet hands.
Follow all lockout/tagout procedures.
Always wear the proper protective equipment for each job.

You are giving a lecture on this issue and you need to ask people on Yahoo about incidents in these locations? I don't think I'd want to attend this lecture. You sound scary.

How about if you simply stick to the most basic safety measures like:

No lose clothing around machinery
Keeping hair tied back
Removing jewelry

Get what I'm saying? You can say a lot without saying much at all.

Or...is there another reason you want to hear about this stuff?

i dunno if this counts but there was a guy that got eaten by a wood chipper near the town i live in. some say his partener pushed him in.
