One time about three years ago. My tooth rotted and I had to have a root canal done, but no dentists were open in the middle of the night. The pain was so bad, I do not know what came over me, but I punched myself in the face. It did not help with the pain.

There's a catagory in SIV (Self-inflicted Violence) talking about this kind of violence. I have not punched myself in the face, but I do know of friends who attempt to take control of their life by controlling their pain-levels through SIV. That may be what you mean. There's also a book "Healing the Hurt Within" by Jan Sutton that provides a great deal of information.

nope, but I did knee myself in the face once. I was once trying to workout doing some high kicks. Well when I kick sometimes my head crouches down for some reason, and I kicked so high, that when my head went down, it hit my knee...at a fast rate...it hurt!

Our little one, hits himself on the head all the time. Apparently the doctor says that it is an attention seeking habit.
