First of all, you couldn't be a good friend if that's what you really want to tell her. Secondly beauty is in the Eyes of the Beholder! Count your blessing that you are not in her shoe, for I'm certain she wouldn't tell you that. If she is really your friend, then appreciate her for who she is, rather than looking at the outside and judging her. Shame on You. It all depends on your degree of closeness. It's something like this : If she can say something about your bad breath and body odor without you taking offense then you can tell her she's ugly point blank... Because you both know you love each other just the same....
I seriously think she already knows though... I mean she does have a mirror at home right?

I think if you told her, even in a subtle way, it would damage your friendship.
As an idea, you could plan a "makeover day" at a spa or with an aestician(sp). It can be a birthday or christmas gift. Maybe she would see how good she looks she may make the change on her own.

A nicer way of telling her would be "it's the inside beauty that matters...." or "everybody starts looking the same when they're 70(ish)".

no need to tell her directly that hurt! Maybe suggest she need a total make over and be kind to pay for the medical cost. lol.

and you call yourself her friend??? let her be! if she wants to believe she's beautiful, allow her. if you're bothered about that, give her a makeover. however, remember "what you think is what you are". believe yourself to be sexy and soon your actions and demeanor follow suit ; )
