From experience, I'd say its a difficult thing to do but can try the following:
Keep your mind busy. You need to have something occupying your mind at all times. It is when you go blank that stray thoughts creep in.
The other thing you can do is sleep. You fall asleep when you're not thinking about anything. You can say a prayer between the time you clear your mind of all thoughts and the time you fall asleep. Good luck.

It is not possible, nor, do I think, desireable to prevent stray thoughts from entering your mind. It is important to recognize those thoughts though, and to keep them from controlling you.
The thought comes, you recognize it, evaluate it, and let it go.
The reason I say you might not want to prevent these thoughts is that they can be inspiration. Creative impulse, scientific discovery, acts of kindness...they are all born of "stray thoughts".
Where do these "stray thoughts" come from? Is someone feeding them to you? Do they come from books, TV, parents... or are they from within you?
