Yes, an OB/GYN can do a more thorough test to develop a more accurate conclusion. There are several things they look for. No test is 100 percent accurate.
In some cultures, virginity is highly prized, and doctors can do restorative surgery to give the appearance of virginity. Countries with a developed medical system have this alternative available.
Addendum: Tell whom? It is illegal for a US doctor to disclose medical information, if that was your real question.

Yes he can, normally he will look to see if the hyman has been broken. However, your hymen can sometimes be broken without inter course so it is not a sure fire way. Also you can not be a virgin and the hymen still be in tact too. So there is no 100% way for them to tell.

Well, they typically check to see if your hymen is intact. If it is, then you're a virgin. If it isn't, then you're not. However, the hymen can be torn for a number of other reasons unrelated to sex (i.e. sports) so this "test" really isn't reliable. I don't know of other ways the gynecologist could figure out whether you're a virgin other than asking you.
