Diversity is a very vague term, so you are far from limited to writing about being a rural white guy. You can also write about academic diversity, ability or disability (maybe you were not the sporty type, or maybe you had different qualities to your siblings), interests, economic experience and beliefs. Remember as American society becomes increasingly diverse, white folks are no longer the vast majority they once were. Have you ever felt like you were in the minority in any given situation? This would be a good experience to write about.
Also, and I'm not sure if anyone has addressed this, the question is really asking you to show what you know about the nature of the school itself. These types of questions are partly designed to see if you can really think things through. You are being asked not only about yourself, but how you will relate to the school's policies and to the student body as a whole. What is their policy on diversity and how would you make a positive contribution to it?
Above all, try to represent a sense of your real personality, not just some bland essay including what you think they want to hear.

write about how you may be different from others in sex, sexual orientation, race, ethnicity, religion, ability/disability, age. There are endless differences between the people in this world. Maybe you are different because you are catholic and your friend is protestant. Talk about differences, and maybe how people who are different should be treated? In the paper, you might acknowledge that you don't have a lot of contact with diversity, but you feel that people who are different from others should still be given respect and treated in a worthy and humane way.
I have written many papers on diversity... some on my personal experience with diversity having been homeschooled, having gone to a public high school, then private college. I also am a christian, which makes me different from other religions. I am a female, making me different from males, giving me a different role in society... should this be the case? if so why? if not, why?, I am straight, what is my opinion on homosexual relationships? how should/will this affect my work? I have been in the minority, and in the majority, and in foreign countries... all of which provide ample opportuntiy for diversity discussion. I celebrate certain holidays because of my American ethnicity, what holidays do friends celebrate that are different? Ramadan (Islam), Kwanza, Rosh Hashana (Jewish), St. Patrick's day (Irish), Cashmir Pulaski Day (Polish) etc.

If it were me, I would write about my international work experience. I'm also white and middle-class, but I've worked in Spain and Italy. I'm unique in that I bring solid experience working successsfully with different cultures.
If I were you, I would probably write about my farm experiences. I would examine three or four "stereotypical" characteristics of farmers and show how I actually embodied those characteristics, and why they would work in med school. For example, people think farmers are "simple". You could elaborate on that and talk about a time you found a simple solution to a complex problem. You saved time and money. Some people think farmers are "traditional". Same thing- talk about how your traditional values like persistance and honesty would make you a good doctor. Having a unified theme- in this case stereotypes- will really pull your paper together.

How about adaptation to diversity and commitment to equality? If you are applying to med school, you may want to think of something along the lines of the following:
No matter where we were born, what we look like, what colour skin we have or what god we believe in we are all essentially equal, but individually diverse.
Read the speech the Jewish gentleman makes in the Shakespeare play The Merchant Of Venice, towards the end of the play when he is trying to cash his bond in court. This will give you a good idea o what I am suggesting.

Try approaching it from a standpoint of versatility... There is MUCH diversity in versatility... Bring back the person-to-person feel of a small farming community to the cold, impersonal and unfriendly big city hospital scene... Dwell on the warm, care-giving aspects of assisting someone in need... Maybe even throw in something about birthing cos and horses... Make a POSITIVE statement and think not only information, but lifestyle...

Talk about diversity in your school. Or how diversity has changed our nation & how that impacts you. Write about stuff you've learned from people of different races. Or friends you have that are other races or even teachers. Make it personal don't just write about the same old stuff people talk about. Martin Luther King gets a little old... for everyone.

What are your qualities, you attributes? Since diversity means difference, It holds to race, ethnicity ,gender social class, age, say some one in there 30s can bring experience into play. Religion, location national orgin. it is valued for its multiplicity of ideas and viewpoints.
