the yellow & red card was introduced to the World Cup in 1970 in Mexico. there was no expulsions in the entire tournament.
Carlos Caszely (CHILE) was the first player in the World Cup history to received the red card against WEST GERMANY at Olympiastadion in Berlin, Germany during the 1st round Group 1 match on 14.06.1974. The referee of that match was Dogan Babacan of TURKEY.
Jose Batista of URUGUAY was sent-off after 56 seconds during the 1986 FIFA World Cup 1st round Group E match against SCOTLAND on 13 June 1986 - a world cup record for fastest red card.

Tim Borowski from Germany was the first player to be substituted in the first round (in the first match, Germany vs. Costa Rica), if that's what you mean. Sebastian Kehl replaced him.
