Actually, I have a birthmark on my middle finger. I would probably just say "birthmark back at ya" (just for fun).

My little girl has a wart on her middle finger and shows it to people all the time which makes me cringe when i see the horrified look on their faces seeing a 4 yr old putting her middle finger up to them!

I would just laugh it off and throw it back at you. For me, the middle finger is not offensive. It's just a stupid gesture that we have been doing since we were kids.
