couple of reasons.
a) lack of activity. If there is not that much use for restrooms separated by sex, then why have them.
b) lack of space. Why build three, while one will do. Yah, three, hers, his and their's.

Because by the time you get to college you should be a big enough kid to realize that boys and girls don't need separate plumbing to pee.

Most of these are there for a parent and a child to do in together.
