It means that the amount of air displaced by the engine is less than 50 cubic centimeters, which is not very much in the first place. A typical 2-stroke 49cc scooter can go about 35-40mph tops.

I own a 93 Harley with the evo 1340 cc motor. Bought it new. Yep, it's fun. I also own a 49 cc E-Ton scooter, it's fun too! The scooter boasts 5.2 horsepower, interestingly, my Husquvaurna, (is that spelled right?) lawn mower also boasts 7 horsepower! Well guess what? I love the Harley, but when it comes to making a few errands or a quick trip to the grocery store I take the scooter. The scooter gets up and gets going with no fuss and unlike the Harley has a kick start if the battery is dead.

In some states, maybe most, a driver's license is not required for a bike less that 50cc.

It means that they are very small.
Less than 50cc is almost like a chainsaw.
