Many comedians, when performing, put on a "persona" that doesn't necessarily reflect how they are in real life (or are an exaggeration of their real traits). Paul Reubens had his Pee Wee Herman character; Bobcat Goldthwait comes off as someone with brain damage; Victoria Jackson plays like she's an airhead (wow, all these '80s references...). Regardless of whether it's for "ratings" or not, Chelsea Handler has chosen the narcissistic, tactless slut for her comedic persona. Considering that she's put on that persona from even before she had a regular gig, I'd guess that it isn't expressly for ratings--it's simply the role that she plays when she's working, and that's what makes her popular among some. No doubt she's a different, more complex person in real life.
