I would get up and move. The person may go wacko if you tell them, maybe even tell you that you are crazy, or deny having lice. Hopefully they will realize they have lice when they get home and do something about it.

If she's a known person ,I would start chatting and as if my girl has that problem and would mention the name for anti -head lice.
If she is a stranger I would say nothing.
