This instills fear within their own group as well as other groups. It is their way of saying, "you had better watch it." It is called a mafia hit; this is their calling card. If it is drug deal that goes bad, the bullet is in the mouth. If it is skimming off the top, it will be bound hands abd a plastic bag over the head.

It means there'll be lots of blood to clean up afterwards. Should it really mean anything? Maybe it's a dramatic action to combo with "Eat this!"
