I don't think you mean Dilantin as this is a medication used to control epilepsy and not pain. You are probably referring to DILAUDID as very potent pain killer.
Dilaudid, like morphine, is a schedule II narcotic used to manage moderate to severe pain (like surgery). The generic or chemical name is hydromorphone HCL. It is available for administration in the form of IV, oral tablets and suppositories.
Several years ago I was a drug sales rep for Pfizer and we learned about pain medications as part or our training. The MD that came in to discuss narcotics (pain killers, sleeping pills, anti-anxiety agents, amphetamines) described dilaudid as the closest thing to heroin legally available to control pain in the USA. Although morphine is the first drug of choice for managing severe pain, this MD preferred dilaudid due to its effectiveness, dosage available (2mg, 4mg, 8mg) and that it is available orally for post pain control when the patient goes home.
Additionally, when my mother was dying of cancer (lung, pelvic and brain) her oncologist prescribed MS (morphine sulfate) first and it worked for a while. Later, the MS was only effective in high dosages so heput here on dilaudid. The 4mg oral strength worked very well for a few months, better then the MS according to my mom. Later the pain became so severe that the doctor finanally had to put her on dilaudid 4mg 3x a day that was injected into her brain by a nurse using an injection pad surgically placed into herbrain.
Her pain was well controled until the day she died and she all was said that the dilaudid was the only thing that eased her pain. If it we me I'd vote for the dilaudid....especially since you have developed a tolerance to MS.

PS Be careful about leaving any pain pills around. Back in 1987 when my mom was on first oral 2mg dilaudid tablets I would go to the US Naval hospital (my dad was a retired Naval officer) and pick up my mom's dilaudid....300 at a time; enough for 1 month. Back in 1987 a single 2mg tablet sold on the street for $20 to $25. So... $20 x 300 equals $900. And the cost to buy them legally at a commercial drugstore; maybe $75 back then. So just be careful whereyou might leave your purse!

I am having a major surgery in a couple of days and have had like 9 surgerues over the last 18 months. I have built up such a high tollerance to iv morphine and want to know what you think of iv dilantin (spelling could be wrong on this medication).

I work in Orthopedics and morphine is the number 1 drug of choice. You might suggest a breakthrough pain reliever such as OxyIR. The important thing to know is that you are entitled to pain control!! Contrary to what you have been told....you have the right to be properly medicated for pain. Most people are unaware of this and think they have to live with pain. Good luck.

What should I avoid while taking phenytoin?
