I have 89 Honda VTR 250cc with 22k miles. 8 out of 10 times when i ride the bike the rpm gauge shows the bike redlining in top gear when I'm just going 55mph. The other times when it seems normal I can go past 80 mph no problem. Maybe the gauge is off? because I still gas it and go faster even when it shows redline. I can tell by the engine sound and it seems like the gauge might be off. Also when i turn the choke all the way down the bike idles at 3-4 RPM.

Sounds like your 'tach is messed up. Dealer-visit time, there...use your ears, if you've heard the engine when it's spinning fast, and you know what that sounds like then let that be your guide. Also, make sure you're really IN 5th gear...good rule of thumb is, if it sounds like you're about to bend valves, don't turn the throttle any harder...

Since the tachometer is cable opperated and not electronic, the tach is most likely defective. If the tach cable was broken, the cable would only turn at the speed of the engine or slower. So if the cable was bad, the gauge would never show a false redline (same thing with the tach drive in the engine).

If the engine sounds fine, then it's the cable, or the gear that runs it, in the front hub. ( Even if it's the cable, you'll need to rectify what's ruined it, in the first place). If the bike seems to be spinning hard, for the speed, then you'll need to adjust your clutch, as it's slipping. Also, you need to adjust your idle speed down to , about, 1000 rpm.

Sounds like the speedo. cable. Get a new one. They get springy with age and don't register right...
