If I report my cell phone as being lost, and give it to my girlfriend to have activated (both with the same company because she lost hers), and i buy a new phone with the insurance discount, would the cell phone carrier be able to tell that my old phone she is having activated was reported by me as being lost or destroyed?

Yes, do not do anything fraudulent with your insurance or with your provider. I sell phones so I would know. Every phone has an imei/serial number that is embedded in the phone and your provider can tell who's using it and the days and times they are using it.
