sorry you paid for a timing belt adjustment 2000 altima all altimas have timing chains. Rough idle on that motor is most likely a bad intake gasket causing an air leak leaning at one cyl. Make sure the black rubber boot from the intake the silver aluminum thing on the back side of the engine is tight and free from cracks and splits air leak too! Buy a can of carb cleaner spray put the extention nozzle on top of the can and spray behind the valve cover, cyl. head to intake manifold area if idle smoots out and speed up you found the air leak. (Average difficulty) for a good technician with a complete set of hand tools. Drain the coolant first Just have the mechanic to triple check to be sure there is no old gasket left on the head or manifold then replace the gasket with factory Nissan Part.

did you check oil levels ? if loil levels are low, have an oil change ... low oil level's can cause "pinging" thats why you idle rough and may lead to engine being blown ..
