Check to see if you can find a listing for him, like in a campus directory or university website.

Unless noted otherwise, I always refer to those who have a Ph.D (in any field, medical or otherwise) as doctor followed by their last name.

Call him doctor. He didn't go through all those years of schooling to be known as a professor.

I'd say it wold depend why are you writing him (if as student, call him proffesor, if anything else doctor)...
