I think the greates comic book villain was the joker.

If you've read anything with the joker going really overboard (the killing joke, some of the IC stuff) you'll know that when he wants something done, he'll get it done. The best example of this is during the Killing Joke comic, when he cripples barbara gordon (batgirl), strips her, takes pictures of her naked, capture her dad (comish. Gordon) and strips and cuffs him and makes him look at the pictures. Thats just messed up .... In most comics hes just the villian that batman lets get away, but in some classic stories, hes arguable the best villain ever

I'm not sure he's alive, but years ago Brainac was a villain in the Superman comics, he was smarter than Lex Luthor. Even though he was locked inside a prison he was able to build a machine that could destroy Superman.

I don't know about THE GREATEST, but my personal favs would be:

Mister Sinister from the X-Men comics (back when it was actually good);

Lex Luthor from Justice League Unlimited cartoon series because they write him so well

Darkseid because he rules his own planet, and his powers are nearly god-like.

Darkseid: The villian so badass, he switched the positions of planets, putting Daxam (Krypton jr.) beneath a yellow sun. Just so he could mind control an entire planet of beings on par with Superman into destroying everything. One of the so called "New Gods", Darkseid is the ultimate plotter. He can go toe to toe with the entire Justice League, and win. Enslaver of worlds, destroyer of hope, murderer of kittens (probably).
