Just ask him.
Say something like:
"Sir... few weeks ago, you said something like I should see some raise coming my way.... I'm wondering if I might expect it in near future..."
If he responds in positive, follow it up with...
"Would it be possible for you to tell me when??"
Remember, he is NOT obligated to give you anything unless he told you in writing. What you thought he said is just that.... you thought... Things like this change daily in some companies. Be ready to accept NO as an answer, and DO NOT harass him, sound negatively, or act negatively. It will affect your prospect in your FUTURE raise.

"Quality's" answer is right on the money. Don't ask to talk about the raise, ask to have him evaluate your performance. Show him that you really want to do well, and if there are any negative areas mentioned during the talk, ask how you can improve in those areas, and thank him for any positive feedback. When the talk is over, tell him you'd like to do this again in a month, so you can discuss improvements you will have made by then. If he says you're doing a great job, it would be fine to casually remind him that you are looking forward to doing well enough to earn that raise you were promised :) If he gives you areas to improve on, then tell him you'll work hard to improve on them so you can earn that raise that he mentioned. Make it about performance, not greed, and it'll go over well. Good luck.

Just ask to meet with him to discuss your performance. During that time would be a good way to casually discuss a raise and get an idea of areas you need to improve in.

You should allow the time to come and go that you were supposed to get the raise in, and then ask him if there is any wrong with your performance since you did not get the expected increase in salary. It shows you are willing to meet company expectations and not trying to badger him about money.

oh gosh, they raised my rent again, i can't wait to finally see a raise, you selfish monkey
ok maybe leave out the monkey part...
