I am planning to write a new yellow page advertisment for my organization. While looking through the yellow pages I notice that most yellow page ads are nothing more than large business cards with the customer's name at the top.

u can put logos or a picture showing what ur company all about. Pictures of people also entertaining

There a book you can buy for making yellow page adds . We used a separate add company to make a add . If you use yellow pages person all the add seem to look the same .

Welll, an idea would be to pay an advertising company or a consultant to come in and give you professional advice.
To track response, ask your customers where they heard about your company.

Do you have a logo or a slogan? When you meet with your sales rep, he/she will help you with making your ad stand out...To track your responses to the ad, get in the habit of asking "How did you hear about us"?
