the yellow cards & red cards was first introduced by British referee Kenneth George "Ken" Aston (1 September 1915 - 23 October 2001) following the incident in the 1966 FIFA World Cup quarter-finals between ENGLAND & ARGENTINA. Jack Charlton was booked by the German referee during that match.

the 1970 FIFA World Cup was the first major football tournament when the yellow & red cards was first introduced. there was no expulsions in the 1970 finals.

Carlos Caszely of CHILE becomes the first person to be red carded in a competitive football match when he received his marching order in the 67th minute by referee Dogan Babacan from TURKEY during the 1974 FIFA World Cup 1st round Group 1 match between WEST GERMANY & CHILE on 14 June 1974. the match was held at Olympiastadion in Berlin, Germany.

I don't know who was the first to get a Red Card ever but the first person to get a Red Card in a World Cup Placido Galindo (PER) vs. Romania in the 54th minute in 1930.
