If they are just surface stains (like soot) there are sponges impregnated with chemicals to scrub them off. If they are white streaks from recent masonry work there are acids to clean that off with a stiff bristled brush. If they are rust there are acid-based chemicals for that. If they are mold bleach will get them off. Paint comes off with paint-scrubbing chemicals. Anything really nasty like grape juice or wine try cleaning it with strong detergents. If all else fails you might need to do some painting over.

I have French limestone surrounding my fireplace that is not sealed. I use a product by TileLab called stone cleaner to remove the soot and it works on both the stone and mortar between it. It also removed some wine someone spilled on the hearth.
