it means you should go see a psychiatrist. it is an insult of sorts, meaning your a nutball. if someone really thought you needed to see a psychiatrist however, most people would be polite and handle the issue sensitively. they would ask you to go see a dr. so when someone phrases it like 'get your head examined' it is pretty much a put down.
