The original character and design of Ronald McDonald, including facial design and costume (featuring "french-fry bag pockets" and "food-tray hat"), were created by Terry Teene and George Voorhees. Voorhees, a professional clown, first portrayed the character for a hired performance at a Los Angeles, California McDonald's. The performer was subsequently legally enjoined from performing as, or exhibiting the likeness of, the character in any form.
First performance, apparently, was 1963

Well, he started out as a ploy to get the south into eating more McDonalds... Originally introduced as "Master Ronald the slave-owning Clown," the execs at McDonald's thought that would create an appeal in the racist south of the 1960s. Then it was suggested why don't we just make him a clown on LSD to appeal to the counter-culture movement that was spreading nation-wide. And overtime it just stuck ... except for his LSD use.

Ronald McDonald was exiled from his highland Scot clan because of his bizarre appearance. He wandered the streets of Scotland and the United Kingdom, eating cheeseburgers and searching relentlessly for the perfect restaurant to suit his unhealthy lifestyle.
Stowing himself away on a potato ship, he arrived in the United States and soon found what he was looking for. Not only did the McDonald's restaurant chain bear his name, but they served good-tasting, extremely unhealthy food.
It was a perfect match.
One day, as Ronald was eating a stack of cheeseburgers in a small McDonald's restaurant in Emporia, KS, a McDonald's executive happened named Grimace spotted him. The restaurant was looking for the perfect representative, and it had found him.
Grimace made a phone call to the Hamburgler at McDonald's headquarters to inform them of his find.
The rest is history.
